The Benjamin Kalkstein & Jay Centifanti Memorial Recognition Awards

What are the Ben Kalkstein & Jay Centifanti Awards?

The Board of Directors of PMHCC, Inc. has established these employee recognition awards in remembrance of Benjamin Kalkstein who died in August 2000 and Jay Centifanti who died in September 2011. Ben and Jay were active members and/or officers of the PMHCC Board for many years. In their honor, PMHCC has established “The Benjamin Kalkstein and Jay Centifanti Memorial Recognition Awards.” This annual recognition is presented to two individuals selected by a committee of the Board.

All employees are eligible. Each Department or Program of PMHCC, Inc. and PMHCC-CTT, Inc. can nominate one person for each award based on their “outstanding contributions.” Each Department/Program Director should establish a selection procedure to identify his/her nominee. A Board selection committee will review all entries and decide on the winners. The winners will receive a cash award ($500.00) and plaque. All other nominees will receive a certificate of recognition and a gift card. In addition, the winners and nominees will be invited to the PMHCC Annual Dinner Meeting; held each year in September. (This year, the Board Meeting was held: 9.12.2018).
